Primary Care Advanced Nurse Practitioner
An excellent opportunity has arisen to appoint two new Advanced Nurse Practitioners to work across the Rhondda Primary Care Cluster.
The successful candidates will act as an expert resource within the Rhondda Primary Care Cluster, providing specialist advice regarding the clinical management of individuals with minor illness or chronic conditions.
Through supporting the delivery of the Care Home Directed Enhanced Service, the role will be key to enhancing the care provided for residents in care homes through a proactive, holistic, coordinated model of care.
The role will directed by the Rhondda Primary Care Cluster and work in collaboration with General Medical Practices, Care homes and the District Nursing Service to meet patient needs.
The post holders will be an experienced primary care nurse, hold the Advanced Nurse Practitioner qualification and be an independent prescriber. They will autonomously manage complex caseloads and take a patient centred and enthusiastic approach to this challenging role as well as an ability to travel between locations.
The Rhondda Primary Care Cluster is a forward thinking cluster which brings together local services involved in health and care across the Rhondda. Working as a cluster ensures care is better co-ordinated to promote the wellbeing of individuals and communities.
This post is fixed term for two years and would be suitable as a secondment opportunity.
