Social Services Single Point of Access
The RCT Single Point of Access Team (also known as SPA) is based in Ty Elai, Williamstown, and provides a remote assessment service, providing information, advice and assistance.
The Single Point of Access (Adult Services) undertake a proportionate assessment by having a “What Matters” conversation, usually over the telephone, during which they identify the outcomes for the individual. The Single Point of Access will then pass the assessment to other teams for a more specialist assessment or commission appropriate preventative or community services.
What problems can they help with?
SPA can assist individuals to access resources to enhance their opportunity to regain, learn or maintain levels of ability.
We can help individuals who have been unwell, suffering ill health and have lost confidence as a result.
Mainly people contact us for additional support when they find it difficult to manage their care and support needs because of disability, illness or frailty or because of advancing age.
We help individuals with:
- A physical disability
- Cognitive impairment or diagnosis of Dementia
- Learning difficulty or disability
- Sensory Loss
- Poor Mental Health
- Caring responsibility.
